01/​05/​2015: miscommunication amongst different locations. Not to say anything of the Dr and tests run IMHO.
Dr is responsive to emails.

From my first phone call to schedule my very first appointment everyone was very kind. The receptionist Nancy is great! She has a very calm & quiet manner about her that immediately relaxes you. Dr. Jacobs is amazing. He too is so easy to talk to & reminds me of my Dad. He explains everything & also brings a sense of calm over what is a stressful situation. Roberta the ultrasound tech is AMAZING!!! A few times she was off when I had to come in & I have to say I was almost sad. The other nurses are good as well who draw your blood, etc. They learned my veins right away & made my blood draws easy. We did 3 IUI’s that were unsuccessful & then on our first round of IVF we were pregnant! Now I am the Mom of a 4 month old baby girl! She’s perfect! The only negative I can share is the main procedures are not done at the CL office. You have to go to another one of their offices. Everyone’s journey to parenthood is different. All I can say is «Stay Calm & Call Dr Jacobs»!!!

I was referred to FCI Crystal Lake after years of trying and one miscarriage. This clinic was an hour away, but I heard so many good things about Dr. Jacobs that we had to try. He was very easy to talk to. Everyone we encountered was professional and supportive. Roberta, the ultrasound tech, was so positive and made us feel comfortable. She makes an effort to get to know every person she sees. We did a lot of traveling for procedures since they are not done in Crystal Lake, but we knew the outcome would be worth it. After 3 unsuccessful IUIs, we tried IVF. I got pregnant after the first try. We now have a handsome 4 month old son. We are truly blessed to have had such a great experience. Dr. Jacobs and the FCI team helped us accomplish our dream of having children when we thought it was impossible. We will definitely be returning next year to try for number 2!

If I could give negative stars I would. I started out at Glenview, then Hoffman Estates and then decided that I’ll give them one last shot at Crystal Lake.
(Main issues at Glenview and Hoffman Estates, COMMUNICATION)
1. Communication is the absolute worst, I keep giving chances for it to improve and it never did.
2. How they run things leads to poor communication and errors. (is this where you want errors and poorly managed)
3. Appears the the majority of staff is either unhappy or just doesn’t care. (Minus the only person who appears to know there job and how to treat people is the ultrasound tech, she is awesome)
4. Procedure, not well communicated, nothing like have an egg extractions being there at 7am and waiting for an hour and half with no explanation why. Several people in and out of your room which appear to be rushing. Procedure room has lovely water marks on the ceiling for your viewing. Room is cold and they don’t offer blankets. After my procedure they don’t tell us I had extra bleeding, they also reviewed everything with the door open so anyone could here (hipa law no no) and while I was leaving headed to the parking lot they forgot that I still had packing in me. YEAH someone forgot this.
So am I surprised that nothing came of my first IVF, for all I know they lost it. WHAT a SHIT SHOW and after for about a week issues with my bladder.
5. Follow up appointment I get a sad face and hug and of course as I leave, once again a communication error, they forget to give me a medication sheet.
This is only a few things that happened, I’m not putting everything in here as I plan on making some formal complaints.
PLEASE Look somewhere else first, they have a lot to work on here before they should get anymore new patients.

After 3 years of trying to get pregnant and multiple miscarriages even through fertility treatment with a different clinic I knew something had to change. I went to meet with Dr. Jacobs and could tell I was in good hands. The staff at Crystal Lake were amazing and Dr. Jacobs really is the best when it comes to treating infertility. After one round of IVF I am now the proud mommy of 1 year old boy/​girl twins. I can’t thank Dr. Jacobs enough and highly recommend FCI. I visited multiple FCI locations throughout my treatment and was always treated well.
