I think these guys are scam artists. Be very careful if going to them. First, before a doctor even saw me, I was offered a list of prescription pain killers, creams, etc. that I could choose from from a list. They said they would submit it to my insurance and send the ones that were approved by my insurance directly to my house. I declined to pick any of these before the doctor gave me a diagnosis. Second, I recently received a $ 1200 bill from a DNA testing lab. I know the day of my visit, all I received was an x-​ray and a look over from the doctor. They claim I signed a waiver for the DNA testing. Even if I did, I feel they were not straight forward with me on exactly what they were doing and what would or wouldn’t be covered by my insurance. Personally, I think it is a scam they are running to take advantage patients that trust in what the office asks them to sign. I would AVOID this doctor at all costs.

We have been thoroughly impressed-​top notch care & caring. Dr. Madsen specializes in minimally-​invasive procedures when possible. His staff is efficient, communicates effectively and follows up well. Dr. Madsen is thorough and communicates technical medical information in a way that the patient can understand. Our care at Forest Park was top tier and my wife’s recovery has been fast and comfortable. I may have a need for Dr. Madsen’s surgical skills soon and would not hesitate based on our experience.
