1 Star for the complete incompetence–
After checking with Humana and finding a provider through the Humana website I decided to claim 4^th Street Dental Group as my place of provider and Dr. Kosydar as my actual dentist. This was after the 5^th call to Dr. Kosydar’s office — confirming that they accept the Humana HMO. I switched during the insurance ‘open enrollment’ period and called yesterday to make an appointment (new patient). That is when the story changed and the receptionist/​office manager explained to me that they are no longer taking that insurance or any HMO’s. She explained that she told Humana but they haven’t taken them off of the website yet. In hind site, I would like to thank the office manager for not staying on top of her duties as I’ve Googled a couple of other reviews and I’m positive I wouldn’t want to go to Dr. Kosydar anyway.
My recommendation is to go to a dentist that provides the services that they advertise.

Proficient, clean and affordable. Dr. Kosydar’s office has been helpful and accommodating.

So rude it is unbelievable. They had to reschedule my appointment and I’m having bone loss in my mouth! I’m having a very traumatic time in my life losing a tooth, and they all treated me horribly. I tried to resolve the situation but they constantly wanted to argue about who was right. I do not recommend this office. I really feel like they did not care about me as a person, but just another dollar. This really ruined my day. I was nice was them the whole time and they were very angry and nasty. So sad.
