I believe Doctor Hamadi to be wholly unethical in his practice. My experience with this man and his staff was extremely bad. He caused me injury, denied it, and then charged me for follow up appointments made in an effort to correct it. He was argumentative with me and accused me of trying to swindle him when I asked not to be charged. With the help of muscle relaxers, 800mg Ibuprofen, and massage therapy, it took 55 days to regain some semblance of normalcy after what he did to me and even after that I still experience muscle soreness daily, but at least it is no longer outright pain and fortunately my medical expenses are covered. For that reason, and that I seem to be slowly recovering, I decided not to file a claim with the state board. But I would strongly suggest you record any conversation/treatment you have with him. It is relevant to note that I was a new client and he injured me on my 3rd visit. I will never go back.
